The Education and Training Directorate provides replacement certificates for those lost in exceptional circumstances such as documented fire, flood or other natural catastrophe. The Directorate also provides transcripts for lost of mislaid certificates.
Proof of identity must be provided at the time of application and should be in the form of a passport, driver's licence, proof of age card or birth certificate. Where the applicant's name differs from the time of their year 10 examinations, a marriage certificate or other legal document showing the name change must be provided.
Certificates and transcripts can be collected by the applicant in person or by an agent holding written authorisation from the applicant (proof of identification will be required in both cases). Alternately, the documents may be mailed to the applicant.
