Name Change Application

Persons whose births are registered in the ACT may apply to have their name changed to an assumed name on the register of births.

In order to change their name in the register of births, an adult must complete a change of name form in accordance with the Births, Deaths and Marriages Registration Act 1997.

To change the name of a child under 18 years of age, the child's parents/guardians must submit the application on their behalf.
