Death Certificates

Death certificates are issued to a spouse, child, parent or sibling of the deceased. If the deceased has no immediate family still living, the certificate can be issued to another person if the following information is provided:

    • written authority of a next of kin;
    • evidence of custodianship/guardianship of the person applying for the certificate;
    • enduring power of attorney from the deceased;
    • evidence that you represent the person named or next of kin (solicitor etc); or
    • evidence that you are a funeral director upon lodgement of the forms or applying for a certificate within two months of registration of the death.
