Persons who wish to formally change their name in the Register are required to complete a change of name application form. This form includes a statutory declaration that is to be completed by the person requesting the change of name and witnessed in accordance with the Statutory Declarations Act 1959.
In some instances where it is not possible to obtain a declaration from the person, a statutory declaration attached to the change of name form signed by the person's solicitor, who knows the person to be one and the same person will suffice.
The Certificate of Title should be lodged with the change of name form or produced by the financial institution.
